SPORTS APPLICATION PROCESSWe are happy to announce that MES now uses an online sports application process.
To register for MES Sports, complete the following steps:
1) Online
- Log into Genesis Parent Portal with your username and password
- Select the Forms tab from the top menu options
- Find the child for whom you will be completing the sports application and select the Sports Registration Form for the upcoming sports season
- Complete all required components of the registration form
- This includes: selecting your sport, medical paperwork acknowledgement, confirming receipt of release forms, emergency contact information, participation agreement, and a digital signature

- Update your answers and finalize the form (Once finalized, you will be unable to go back and change your responses)
2) Medical Paperwork
- All student-athletes interested in trying out for MES Sports must complete and submit the following medical forms:
- Full Physical Evaluation Packet
- Physical paperwork must be completed within 365 days of the season start date. If the last physical date was prior to 365 days, a new physical must be completed and submitted.

- Health History Update Questionnaire
- A Health History Update must be completed if the student-athlete's physical examination was more than 90 days from the official start of the sports season.

Based on the student-athlete's health conditions, the nurse's office may require additional medical forms to be completed prior to clearance. These include: Medication Authorization Form; Asthma Action Plan; Food Allergy Plan; Seizure Action Plan; Diabetes Medical Management Plan) Not completing these forms may delay your student-athlete's clearance to participate.

Medical Paperwork can be dropped off to MES main office between 8:00am and 2:30pm Monday-Friday.
