    SAFETY AND HEALTH PLAN

EXPECTED BEHAVIOR IN ALL LABORATORIES AND CLASSES

Students are expected to conduct themselves with the maturity and responsibility of any person who works in a hazardous area, using potentially dangerous machines and equipment.  These responsible actions include complete cooperation with the teacher by following all safety and health instructions.  This cooperation precludes such actions as horseplay, unauthorized use of materials, tools, machines and equipment, distracting and disrupting others while they are working or any other unsafe practices.

Any student who consistently demonstrates that they present a safety hazard to themselves or others students or damage school property shall be removed from that class and result in a zero.

    Procedure for student violation:

First offense: Teacher gives verbal warning indicating the nature of the incident.  Student must sign and date written warning.  Student may face detention and loss of lab privileges.

Second Offense: Teacher gives a written warning describing the nature of the incident, which must be signed and dated by the student and parent.  A meeting with supervisor, AP, guidance, or parent may be warranted.  Discipline action will be determined and possible removal from class will be considered.

Third Offense: Teacher gives written notice of situation and nature of the incident.  Student and parents must sign and attend a mandatory meeting with the teacher, guidance counselor, and AP.  Student may be removed from the class permanently and credit for the course may be withheld.

I HAVE READ THE ABOVE POLICY AND AGREE TO ABIDE WITH IT IN ALL ASPECTS.  NO STUDENT SHALL PARTICIPATE IN ANY LAB WITHOUT STUDENT AND PARENT SIGNATURE.
