SPORTS PHYSICALS FOR 2012/2013 SCHOOL YEAR

Pursuant to N.J.A.C. 6A:16-22, the New Jersey State Interscholastic Athletic Association (NJSIAA) has recommended that each student’s medical examination be conducted at the “medical home” of the student. As a result, students will need to obtain individual sports physicals from their own physician to be considered for participation in sports at Manasquan High School. Therefore, the District will no longer schedule sports physicals at the high school. 

It is recommended physicals be scheduled well in advance of the start of the season.    The Manasquan High School History and Physical Forms must be used. Packets will be available by May 2012 in the MES and MHS main offices, nurse’s office, and the athletic director’s office. Forms are also available on our website. Completed forms must be returned to the nurse’s office no later than July 15, 2012.  No student-athlete will be allowed to participate in any athletic activity until the proper medical forms have been completed and reviewed by the school nurse, the athletic department and approved by the school physician. 

 Athletic physicals are valid for 365 days from the date of the original physical. Medical Updates are required if the original physical was conducted more than 60 days before the first official practice date. This is to ensure that there has been no change in the student-athlete’s condition. It is important that all time-lines are met. Athletes may not participate until paperwork is submitted, recorded and processed.  Late submissions may result in the athlete missing practices and/or games.
