IMPORTANT INFORMATION REGARDING SPORTS PHYSICALS
 
The Physical Exam (Part B) will be offered free of charge to Manasquan students on the following days at MHS

Please note: there will be NO MAKE-UP SESSIONS!
 
Students must bring all other 5 parts of the Sports Application(Part A: Health history, application, conduct agreement, steriod testing policy, and emergency contact card) with them to the Physical Exam. 
 
Students must wear shoes and bring eyeglasses/contacts (if required) for vision test.  Girls should wear a two piece bathing suit under clothing.

Mrs. Bontales is a Certified School Nurse
 
Mrs. Bontales has a Masters of Science degree in School Nursing from Monmouth University, West Long Branch, and received her Bachelor of Science degree in Nursing from Wagner College on Staten Island.
 
Prior to entering School Nursing, Mrs. Bontales worked in the following clinical areas, Operating Room, Emergency Department, Infection Control, Geriatric Nursing, Alcohol and dual substance abuse D-Tox, Quality Assurance and Utilization Review.
 
Mrs. Bontales served on the New York City Aids Task force and the American Red Cross Disaster Action Team

Mrs. Tellone- McCoy is a Certified School Nurse
 
Mrs. Tellone-McCoy has a Masters of Science degree in School Nursing from Monmouth University, West Long Branch, and received her Bachelor of Science degree in Health and Human Services with emphasis on Mental Health.
 
Prior to entering School Nursing , Mrs. Tellone-McCoy worked in the following clinical areas, medical surgical nursing , othopedics , oncology, rehabilitation and psychiatric emergency room as a certified psychiatric screener.  Mrs. Tellone- McCoy also was employed at UMDNJ for many years as a Health and Wellness Educator for the Technical Assistance center and the Center for Excellence in Psychiatry.
 
Mrs. Tellone-McCoy is the advisor to the Health Careers Club here at MHS.
 
 
Both Mrs. Bontales and Mrs.Tellone-McCoy are AED and CPR certified and hold membership with Monmouth County School Nurses Association, NJ State School Nurses Association and the National School Nurses Association.
